Advance Care Plan Workshop with Q&A

This 90 minute instructional webinar will encourage participants to think about having conversations about Advance Care Planning. Step-by-step instructions include showing the participants how to set up an online advance care plan account and how to begin building an advance care plan online.  Starting at 6:30pm, the presenter will dedicate 30 minutes to a Question and Answer forum.
